About Records 1-2-3

Records 1-2-3 Template System was developed over 5 years in the late 1990’s by David W. Vinson, M. D. specifically for the Family Practice office environment. Dr. Vinson is board certified in both Emergency Medicine and Family Practice and has practiced medicine for more than 20 years. He created this template system first to meet the critical need in his own office for a better solution to document patient encounters, and has now made it available to Family Physicians who are using it nationwide.

The Records 1-2-3 Template System consists of a comprehensive yet manageable number of templates and supporting forms. The entire system is completely paper-based and very simple to implement and use. No computer is required! No computer training is needed! Anyone with access to a copy machine can produce templates and use the system. Records 1-2-3 is truly a “no-tech” solution that works to produce excellent and thorough documentation.
