The Problem List

The second major component of the Records 1-2-3 Template System is the Problem List. This redesigned and improved version of the “flow sheet” that many offices use allows the doctor to gather all the pertinent clinical information on the patient in one place and track it over time. The Problem List is an integral part of the system and, if used properly, can greatly improve physician efficiency and productivity.

The front page serves three basic functions

1. Listing of major medical problems and testing interventions
2. Chronological listing of each office encounter or decision point
3. Listing of all medications, linked to the visit prescribed

The back page also serves several vital roles

1. Tracking abnormal or important lab values.
2. Past medical history
3. Social History
4. Family History

The Problem List simplifies documentation and makes it quick and easy to see what the patient has been seen for and what medicines they were treated with on previous visits.
